What is Wave Front Guided or Custom LASIK all about?

Glasses, contact lenses, and traditional LASIK were all designed to correct myopia (near-sightedness), hyperopia (far-sightedness), and/or astigmatism. (See descriptions on this same web page.) However, there are many other types of optical aberrations of the eye which can not be corrected by traditional means. These aberrations, termed higher order aberrations (HOAs) may affect visual acuity and the overall quality of your vision.

Everyone has a unique set of HOAs just like a "fingerprint" of the optics of the eye. These aberrations are measured with instruments that detect variations in a wave front that is projected into the patient's eye by very sensitive diagnostic instruments. Through computer analysis of the reflected wave front, the higher aberrations can be detected and measured. When this information is transmitted to the excimer laser's computer, the laser can then correct or reduce the patient's unique pattern of HOAs. This whole process is called Wave Front Guided LASIK. In our office we simplify this by calling it "Custom LASIK" or "Custom IntraLASIK" depending upon whether the patient has traditional LASIK with microkeratome blade creating the corneal flap, or IntraLASIK where the IntraLase laser creates the flap.

Using the VISX laser, FDA studies have shown a significant improvement in the outcome of LASIK eye surgery with wave front guided procedures. Historically, traditional LASIK provides approximately 70% of patients with 20/20 vision or better without glasses or contacts. With wave front guided LASIK, FDA studies have shown that 94% of patients had 20/20 or better uncorrected vision six months after surgery and 98% had 20/20 or better after 12 months. In addition, studies have shown that in some patients, uncorrected post operative vision was actually better than best corrected vision before surgery. Besides better visual acuity outcomes than traditional LASIK, wave front guided LASIK has also provided many patients with better outcomes in terms of their quality of vision including better night vision. However, it is still important to understand that no one can guarantee the outcome of your LASIK or IntraLASIK surgery, even with a wave front guided procedure. Although the risks of complication are low, there still are risks to any surgery.
